How can I request a copy of my transcripts?
You must send the Registrar's office the following information (print and complete this form if you wish):

  • Full Name (maiden name if married female student)
  • Social Security Number
  • Last Date of Attendance
  • Current Address and Telephone Number
  • Complete address where the transcript is to be sent
  • $5.00 fee for each transcript
  • Signature of the student on the letter (cannot accept requests from a parent, spouse, business, etc.)

These transcripts will be placed in an envelope which will have stamped on the front a statement indicating that if the envelope is opened, the transcript becomes an Unofficial copy.

Will the Registrar's Office accept faxed requests for transcripts?
Yes. All of the information listed above must be included.

Will the Registrar's Office accept requests for transcripts in the form of a telegram?
No, because we must have the signature of the individual.

Will an unofficial copy of a transcript be sent to a student (or former student) when that individual makes the request by phone?
No, the Registrar's Office must have the request in writing.

Will the Registrar's Office fax a transcript and, if so, is there an additional charge?
Yes, an unofficial copy of a transcript can be faxed, and there is a charge of $5.00.

Will the Registrar's Office send a transcript via Federal Express (or other means of express)?
Yes, however, we must have first received from the student payment of the Federal Express charge. We will not send the transcript and bill the student for the Federal Express charge.

Will the Registrar's Office mail a transcript prior to having received the $5.00 charge for the transcript?
Yes, a bill will be sent with the notification that the transcript has been sent.

Are we allowed to report grades over the phone?
No. According to the Federal Educational Rights and Privacy Act of 1974, we are not allowed to report grades over the phone because we have no way to identify the caller. If a student comes into the office and asks to see his/her grades, he/she must show proper ID.
