The form requires the advisor's signature; and in the case of an add, the instructor's signature. Declaration of Major forms Used after you have decided on a major area of study. Forms are available in this office; will need to take this to the office of the department chairperson for purposes of completing form and assigning new advisor. Declaration of Minor and Declaration of Concentration forms are also available and will be used in the same way as the Declaration of Major Form. Pass-Fail form Used to notify Registrar's Office of your intention to take a course based on a pass-fail designation rather than a letter grade. Audit form Used to notify Registrar's Office that you will study course material and receive a designation of audit for this course instead of a letter grade.
